No. A referral must be initiated by a GP, ANP or ACP. The patient expresses a preference for Harley Mind Care, but you as the clinician must submit the referral form.

You do not need to be their registered GP, but the referring clinician must be a qualified GP, ANP or ACP. Locums and sessional GPs can refer. Please ensure the patient's registered GP practice details are included on the form.

Yes — the patient must express a preference for Harley Mind Care to you as their GP. They can request this at any appointment. You do not need written evidence of the request but it is good practice to note it in the records.

A GP, ANP or ACP completes the GP Referral Form (available to download or complete online at harleymindcare.com/right-to-choose-gps), attaches a Summary Care Record or medications list, and emails the completed referral to kmicb.harleymindcare.adhdautism@nhs.net. eRS submission will be available shortly.

Current wait from referral acceptance to assessment appointment is 2–4 weeks for all service types (August 2026). Wait times are reviewed monthly — current figures are published at harleymindcare.com/right-to-choose-gps. Priority is given to Category 1 (urgent) and Category 2 (high clinical need) cases.

No. NHS Right to Choose referrals are funded by the commissioning ICB (NHS Kent and Medway ICB). There is no cost to the patient and no cost to the referring practice.

Yes, but a copy of the original diagnostic report must be included with the referral. The assessment must have been conducted in line with NICE guidelines (NG87). A discharge letter from the current prescribing provider is required if referring directly to titration without a new HMC assessment. The NHS will not fund a second full diagnostic assessment where one has already been completed.

Your patient will be triaged within 5 working days and contacted directly by our team to complete pre-assessment screening and book their assessment appointment. You will receive a confirmation of referral receipt. Following assessment, a consultant-signed clinical report is sent to you and to your patient.

If a patient is not suitable for online assessment, contact us at hello@harleymindcare.com to discuss. Face-to-face assessment may be possible in certain clinical circumstances — we will advise on a case-by-case basis.

We are actively expanding nationally and in discussion with ICBs across England. For out-of-area patients, please complete Sections 1–4 of the referral form and submit to the same NHSmail address — we will contact you once arrangements with the relevant ICB are confirmed. Out-of-area patients are not billable under the K&M contract.

Dose changes and medication switches remain HMC's responsibility under shared care. If your patient requires a dose adjustment, please contact us. GPs under shared care are responsible for repeat prescribing at the agreed dose only — not for clinical medication management.

Under the NHS Choice Framework and NHS Standard Contract, patients in England who have been referred by their GP for a mental health assessment — including ADHD and autism — have the legal right to choose their provider. As the referring GP, you have a legal obligation to inform eligible patients of this right and to act on a valid request. Harley Mind Care holds a formal NHS Right to Choose contract with NHS Kent and Medway ICB, effective 1 August 2026.

Shared care is a voluntary arrangement under which ongoing ADHD prescribing transfers to the GP practice once the patient is stable on medication. HMC will contact you to discuss and issue a Shared Care Agreement at that stage. If your practice declines, HMC will retain full prescribing responsibility — there is no obligation to accept. Please indicate your practice's preference on the referral form.

Under shared care your practice would issue monthly repeat prescriptions for the agreed ADHD medication and dose, and monitor blood pressure and heart rate at least annually. HMC provides the annual medication review — the patient must attend this for prescribing to continue.

Harley Mind Care is not a crisis or emergency service. If your patient is presenting in active crisis, please direct them to call 999 (emergency) or 111 (urgent non-emergency), or attend their nearest Emergency Department. Do not submit a referral if the patient is in active crisis — address the immediate risk first. Our suicidality risk screening on the referral form provides clear guidance on when not to refer.

Adult patients (18+) registered with a Kent and Medway GP who are suitable for online assessment. The referral must be for adult ADHD assessment and/or adult autism assessment, or ADHD medication titration or annual review. Yes. A consultant-signed clinical report is sent to the referring GP and to the patient following the assessment. The report includes a full assessment summary, ICD-10 diagnosis (where confirmed), clinical rationale, and treatment recommendations.
